Connects homeless individuals and families to housing

Homeless Management Information System (HMIS)

Administration of HMIS system for 23 county area in West

Decatur County Rental Assistance Program

Permanent Supportive Housing program for Veterans and

CoC Rapid Re-Housing Program

Serves literally homeless disabled households with Rapid Re
Housing and SOAR
 

 

Addressing Healthcare Among the Homeless Program

Serves literally homeless disabled households with Rapid Re

Unsheltered and Rural Set
Asides

Permanent Supportive Housing and case management for literally homeless with
disabilities.  Set to begin August 1,
2023

 

Support Services for Veteran and their Families (SSVF)

Provides case management and temporary financial assistance
to homeless and at risk of homelessness throughout the 23-county area of West
Tennessee
.

 

Benton County Community Resource Center

A facility that houses several agencies and offers
referrals to needed resources.
 

 

Dyer County Service Center

Focuses on housing and employment for the homeless in Dyer
County.
 

 

Emergency Housing Voucher (EHV) Program

Provides
clients with housing choice vouchers through 2030, ensuring needs are met to
keep clients from experiencing homelessness.

 

Eviction Prevention Program (EPP)

Provides
assistance with rental arrearage to avoid evictions.  Set to begin August 15, 2023

 

HOME ARP Supportive Services
Provides case management and financial services to assist households in
achieving self-sufficiency.

 Permanent Men’s Shelter- City of Jackson

Provides no-barrier emergency housing services and case
management to single men in a non-congregate setting with the priority given to
the literally homeless population.  Program expected to be operating in the Spring
of 2024.
